You can apply in person, by mail, over the phone, or online. Eligibility and enrollment services are provided by the Alameda County Social Services Agency.

If you wish to apply in person, you may go to one of the Alameda County Social Services Agency public contact offices Monday through Friday between 8:30 am and noon and from 1:00 pm - 5:00 pm.

By Mail

To apply for Medi-Cal by mail, please call 510-777-2300 or 1-800-698-1118 to request a Medi-Cal mail-in application and instruction booklet.

New Medi-Cal Rules for Seniors & People with Disabilities in Alameda County

Most seniors and people with disabilities who see their health care providers on a "fee-for-service" basis will be required by the State to enroll in a Medi-Cal managed care health plan. You must enroll in a health plan before or during your birthday month, between June 1, 2011 - May 2012.
