We are pleased to offer Alliance CompleteCare (HMO SNP), a Medicare Advantage Special Needs Plan. Alliance CompleteCare was designed for people of all ages who live in Alameda County, California, and are dually eligible for both Medi-Cal and Medicare.
We created Alliance CompleteCare to make it easier for you to get the health care you need and service you can trust. Learn more about eligibility.
With Alliance CompleteCare, you can choose your own primary care provider (PCP)—and count on the ease of having your own personal Alliance Care Advisor. Your Care Advisor will make sure your questions are answered and will help you coordinate your care.

Alliance CompleteCare accepts new members all year round! To request an in-home appointment, please contact us. An Alliance CompleteCare sales specialist will meet with you and explain the plan in more detail.

Grievances and Appeals
Alliance CompleteCare strives to provide quality service and care. However, if you ever have a complaint as a member of Alliance CompleteCare, we urge you to call your Care Advisor right away. Here is more on How to File a Complaint.

Medicare evaluates plans based on a 5-Star rating system. Star Ratings are calculated each year and may change from one year to the next.
Please note that plan benefits, formulary, pharmacy network, premium, and/or co-pays may change on January 1 of each year. Alliance CompleteCare (HMO SNP) is a coordinated care plan with a Medicare contract and a contract with the California Medicaid program. Enrollment in Alliance CompleteCare (HMO SNP) depends on contract renewal.
