Diabetes Prevention Program (DPP)

The Alliance is now offering the Diabetes Prevention Program (DPP) with our partner, Solera Health. DPP helps participants who are at risk for type 2 diabetes adopt healthy habits and lose weight. This program is no cost for eligible Alliance members.

The year-long program begins with 16 weekly lessons, followed by monthly sessions for the rest of the year. The program follows a curriculum approved by the Centers for Disease Control and Prevention (CDC). Alliance members can choose from digital or in-person program formats.

How to refer your Alliance patients:

  • Complete and fax the DPP Referral Form
  • Alliance members can self-refer by calling:
