Case and Disease Management Program

The Alliance Case and Disease Management Program is designed to assist in managing the care of medically complex members by coordinating services that will ensure the improvement of patient outcomes and overall member satisfaction.

Providers may use the Case Management Programs Referral form to refer members to the Case and Disease Management Program.

The Case and Disease Management team include:

  • Registered Nurses
  • Health Navigators
  • Psychosocial Professionals (through Beacon Health Strategies)

The Case and Disease Management Programs:

  • Complex Case Management empowers members to take control of their health care needs across the care continuum by coordinating quality health care services in collaboration with their providers, including the optimization of benefits through a realistic, cost-effective, and timely case management plan.
  • Disease Management provides services to children (ages 5 - 11) with Asthma, and adults with Diabetes. The members who are at the highest risk are enrolled in Disease Management to determine if they need assistance in managing their health care.  These programs are designed to empower individuals to better manage their disease and prevent complications.
  • Care Coordination supports members in navigating through the health care system including referrals to community resources.
