Alliance CompleteCare (HMO SNP) includes medical, dental and vision coverage, plus other services designed to contribute to your good health and well-being.
The Medical Benefits Chart in the Evidence of Coverage booklet provides complete information on all of your benefits and covered services, including conditions for limitations and exclusions.

Alliance CompleteCare is very pleased to offer dental benefits in 2013 to our members. We have chosen LIBERTY Dental Plan to provide these benefits to you. LIBERTY has a large network of dentists that you can see. Your dental coverage includes routine care and cleanings, X-rays and fillings, crowns, bridges and dentures. You can also visit the LIBERTY Dental Plan Website for Alliance CompleteCare members. This site will help you find a dentist, refer a dentist to LIBERTY Dental Plan, and find a list of benefits and copays. Clicking the link above will take you away from Alliance CompleteCare web page to an external site managed by Liberty Dental Plan.
High Risk Care Management (HRCM) Program
Alliance CompleteCare identifies and contacts HRCM Program candidates who meet certain criteria. Members who want to be assessed for the program may contact the Care Advisor Unit.
Our HRCM Program is designed to:
- Help the member move toward self-reliance and the best use of health care services
- Help to maintain the member’s quality of life
- Provide support and act as a resource to the member and provider;
- Help the member coordinate benefits and services
- Monitor the plan of care and services to ensure quality, timeliness, appropriateness, and cost effectiveness
We care about your health and well-being. Alliance CompleteCare offers a range of services, including a member portal with access 24 hours a day/seven days a week, transportation for medical appointments, and interpreter services.
