Alliance Working for Antibiotic Resistance Education (AWARE) The purpose AWARE, a project of the California Medical Association Foundation, is to reduce the unnecessary use of antibiotics and reduce the prevalence of antibiotic resistant bacteria in California. The project has developed a wide array of tools for medical providers and patient information pieces. Prior Authorization (PA) Request Form (pdf) Formulary Review The Pharmacy & Therapeutics Committee meets quarterly to review the formulary, drug utilization patterns, and establish guidelines, protocols, programs, and procedures that help ensure high quality, cost-effective drug therapy. Formulary changes are updated quarterly and posted on this Website. Providers can submit a Formulary Review Request Form (see below) to recommend consideration for review of a drug.
