Healthy Families Program

Children from birth to 19 years, who are U.S. Citizens or legal residents, and have family income below 250% of the Federal Poverty Level Guidelines may be eligible for the Healthy Families Program.

Alliance Group Care : Designed for In-Home Supportive Services (IHSS) home care workers in Alameda County.

IHSS home care workers may qualify for Alliance Group Care through the Public Authority for IHSS.

To find out if you are eligible for Alliance Group Care, please call the Public Authority for IHSS between the hours of 8:30 a.m. - 4:00 p.m. at (510) 577-3551 or visit their website at http://www.ac-pa4ihss.org/content/72.asp to learn more.

Once you become a member of Alliance Group Care, Alameda Alliance for Health will send you a new member packet and an Alliance member identification card before your effective date.

Alliance CompleteCare
You can join Alliance CompleteCare if you are eligible to receive Medi-Cal and you are entitled to Medicare Part A (hospital insurance) and enrolled in Part B (medical insurance). You must also live in the plan’s service area, which is Alameda County.

To verify your membership eligibility with the Alameda Alliance for Health, please call Member Services at (510) 747-4567, 8 am — 5 p.m., Monday through Friday.
